Congenital malformations stemming from cleft lip and palate, a malformation resulting from the non-fusion of embryonic structures, lead to various morphological consequences, including varied malocclusions that affect the teeth and face. The objective of this study is to explore the possibilities for combining orthodontic treatment with orthognathic surgery to correct defects associated with cleft conditions. The type of orthodontic appliance, installation method, treatment duration, and planning of dental and orthopedic movements must be carefully planned for the success of orthodontic treatment in patients with cleft conditions, and these topics are discussed in this study. As part of the methodology, a case study of a 22-year-old female patient with a complete pre-foramen cleft on the left side, an incomplete cleft on the right side, and an incomplete post-foramen cleft, who was under the care of the Centro de Atendimento Integral ao Fissurado do Complexo Hospitalar do Trabalhador (CAIF/CHT) team, was presented. The patient received multidisciplinary and comprehensive care since she was two months old and underwent various surgeries and treatments to correct the defect and its consequences. The interdisciplinary treatment was completed after 14 years, and the results were analyzed to assess the effectiveness of orthodontic management in correcting the aesthetic and functional deviations from normality associated with cleft lip and palate.
